Dear Patients and Families:

As the Center prepares to administer the annual influenza vaccine, we have begun sending electronic consent forms to patients or their responsible parties for whom we have current email addresses. We ask that you review, complete and sign the flu consent, submitting it electronically at your earliest convenience. While we recommend flu vaccination for all patients who are able to receive it, the consent form includes an option to decline the vaccine, as well. If you have not received an electronic consent form via email, please feel free to contact Dale Jeffries, Director of Nursing, at 571-248-6100 to provide an email address or make other arrangements to complete the consent form.
